Enliven Services available in Rotorua
In Rotorua we are contracted by Lakes District Health Board to support older people with low to high needs who have been referred to us by the local Needs Assessment and Service Co-ordination (NASC) agency.
Home-based, restorative support
Enliven's home-based restorative support services are tailored to meet the needs of older people (65+) who are living in their own home.
After consultation with each client individually tailored programmes of support are developed foryou by one of our Service Co-ordinators. The restorative programme that results invariably involves the integration of physical activity. Services, which can be delivered up to three times a day, seven days a week, are provided by highly trained Key Support Workers.
There is no fixed list of services but a package of support mayinclude:
- Goal-based planning, individually tailored to meetyourneeds and what is important toyou
- Rehabilitation to assistyoutoregain skills, mobility, confidence
- Activities to ensureyoustay as independent asyou can
- Physical exercises based around daily living activities to keepyouphysically active
- Practical support to keepyour household up and running
- Personal care based onyour individual needs
- Regular liaison with doctors, nurses and other health professionals to make sureyouare getting the very best support and the help that is right for you.
Transition Programme
The transition programme provides a solution for older people who want to live as independently as possible in their own homes again after a short period in hospital and/or rest home.
As part of this programme we will actively work with youwhileyouare in residential care, assistingyouto rebuild your capability and confidence to a level required for yourreturn home. This involves developing an individually-tailored programme of support reflecting your needs and what is most important to you.
The aim is to supportyou tomake the transition back home and then to provideyou with the supportyou need subsequently.
Post-discharge support
Our post-discharge service is aimed at supporting people for the first four to six weeks after hospitalisation.
As with the other Enliven services the support delivered is individually tailored to each clients needs and aimed at ensuring people can successfully return home after a hospital stay. Support may include assistance with personal care, household tasks, meal preparation and shopping.
Clients are regularly reassessed and the level of support they receive is reduced over time, as appropriate.
The aim is to ensure that four to six weeks after being discharged from hospital clients are ready and able to live independently again, where possible.
