Home-Based, RESTORATIVE SUPPORT

Enliven's home-based restorative support services are tailored to meet the needs of older people (65+) who are living in their own home.

After consultation with each client individually-tailored programmes of support are developed for them by one of our Service Co-ordinators. The restorative programme that results invariably involves the integration of physical activity.

Services, which can be delivered up to three times a day, seven days a week, are provided by highly trained Key Support Workers.

There is no fixed list of services but a package of support may include:

• Goal-based planning, individually tailored to meet clients' needs and what is important to them
• Rehabilitation to assist an older person to regain skills, mobility, confidence
• Activities to ensure clients can stay as independent as they can
• Physical exercises based around daily living activities to keep clients physically active
• Practical support to keep clients' households up and running
• Personal care based on individual needs
• Regular liaison with doctors, nurses and other health professionals to make sure our clients are getting the very best support and the help that is right for them.

Enliven's restorative home-based support services are presently available in Auckland, Hamilton, Tauranga,Rotorua and Taupo.

Please note: Our services are funded by various district health boards, and are therefore free to clients. However, in order to receive Enliven Positive Ageing Services older peoplewill need tobe referred to us by thelocal Needs Assessment and Service Co-ordination agency.
